The Athletic Training staff at the University of the Southwest provides health care services to USW student-athletes. The Athletic Training offices are located in the Jack and Mabel Maddox Student Life Center, Room 105. They provide athletic training services that include:

•     first aid,
•     injury prevention,
•     evaluation,
•     rehabilitation,
•     reconditioning
•     medical referrals

Please click here to download the Physical Forms. All of the forms must be filled out completely, and the physical must be signed only by an M.D., or D.O per the request of our insurance carrier. Please bring or fax a copy of the front and back of your health insurance card, as you cannot practice or participate unless you are cleared with the required paperwork.

To fax copies of your health insurance, put the "Attention" to either your Coach or Head Athletic Trainer, Steve Burback. Please make sure to write the name of the student athlete somewhere on the fax.

USW Athletic Training Staff

Steve Burback ATC/LAT, enters into his eighth year with the Mustangs staff as Head Athletic Trainer. Preceding becoming the Head Athletic Trainer with USW, Burback worked with the Colorado Venom of the National Indoor Football League (NIFL). Prior to joining the Venom's staff, Burback was with HEALTHSOUTH, a spine and sports medicine rehabilitation clinic in Colorado. He also served, as the Head Athletic Trainer for Bear Creek High School, in Denver.

Burback received his Bachelor's degree in Political Science/ Public Administration, from Colorado State University in Ft. Collins, and Bachelor's degree in Athletic Training from Metropolitan State College of Denver. He is currently finishing the requirements for his Master's in Education Administration. Burback is originally from Denver, Colorado. He has three daughters and two grandchildren.
